This page provides a clear, ongoing reference to the student release forms that parents complete annually for their student(s). The items are listed below so parents and guardians can easily review them at any time. Should a parent want to change their submissions, they would log into ParentView and make those adjustments.
Please read each item carefully. Unless you choose to refuse consent or “opt out,” personally identifiable student information may be released as described, and your student may be considered for the programs listed.
If you choose to refuse consent, your student’s personally identifiable information will not be released, and they will not be considered for certain programs.
If SUSD does not receive this form within fourteen (14) calendar days of your receipt, consent will be assumed for the release of student information and participation in the identified programs.

Scottsdale Unified School District is committed to honoring the rights of parents and promoting parental involvement across the District. The Governing Board recognizes the fundamental rights of parents to direct the upbringing, education, and care of their minor children.
