Immunization Requirements for School Entry
2008-2009

Statutes/Regulations:
A.R.S. §15-871 and §36-672
A.C.C. R9-6-701 and R9-6-707 Tables 1 and 2 - Immunization schedules

Preschool entry:

  • Diphtheria, Tetanus, Pertussis (DTaP or DPT or DT): at least 4
  • Polio (OPV): at least 3
  • Measles, Mumps, Rubella (MMR): 2 doses
  • Hepatitis B (hep B or HBV): 3 doses (or series started and next dose not yet due)
  • Hepatitis A (hep A or HAV): 2 doses (or series started and next dose not yet due)
  • Haemophilus Influenzae type b (Hib): 3-4 doses with 1 at 12+ months -or- 1 dose at 15+ months
  • Varicella: 1 dose or history of chickenpox disease1

Kindergarten, 1st thru 12th grade entry:

  • Diphtheria, Tetanus, Pertussis (DTaP, DPT, DT or Td): at least 4 doses
  • Tetanus, Diphtheria & Pertussis (Tdap): 1 dose required for 6th grade students who are 11 yrs and older and have had 5 years since last DTaP, DTP, DT or Td
  • Polio (OPV): at least 3 with one dose at age 4 or older
  • Measles, Mumps, Rubella (MMR): 2 doses
  • Hepatitis B (hep B or HBV): 3 doses (or series started and next dose not yet due)
  • Varicella: 1 dose -or- history of chickenpox disease (required for grades 6 & 12 in 2009)

NEW!!! Meningococcal Vaccine (MV) is required for 6th grade students who are 11 years of age and older.

NOTE: 2008-09 school year will add 4th & 10th, etc until required for all grades in 2010.

1 - Parental statement of the child having chickenpox is acceptable. No doctor’s statement is required.

2 - IMPORTANT! As of May 2005, the Arizona Department of Health Services is highly encouraging administration of Tdap (tetanus, diphtheria and pertussis) to children 10-18 years of age rather than Td (tetanus and diphtheria only). See your healthcare provider for more details.

The Arizona School/Child Care Immunization Law allows exemptions for medical reasons or if immunizations are contrary to the personal/religious beliefs of the parent or guardian. Medical exemption requires an exemption form, signed by both the physician and parent, specifying the reason for and duration of the exemption and submitted to the school. Forms are available at each school.

NOTE: Do not confuse HIB with HBV, the Hepatitis B vaccine. Haemophilus Influenzae type b (Hib) is an infant vaccine required for preschool entry only.
