SCOTTSDALE DISTRICT
HIGH SCHOOL ATHLETIC ELIGIBILITY
WORKSHEET

  1. PHYSICAL CARD - Current physical given on or after March 1 of current year. (See A.I.A. By-law 15.6 for more detail )

  2. PARENT CONSENT CARD - Completed and signed by parent or guardian.

  3. BIRTH CERTIFICATE - Copy on file for age limit. If a student becomes 19 years of age after September 1 he/she is eligible to compete for the remainder of the year.

  4. GRADES FROM PREVIOUS SEMESTER OR NINE WEEKS - Students must have a passing grade in all classes taken during the previous semester or nine weeks. The number of classes from the previous semester is NOT a criteria; only when the student is on an athletic team is the number of classes a factor.

  5. NUMBER OF CLASSES DURING THE SEASON OF COMPETITION - A student must be enrolled in a minimum of five credit courses or the equivalent the first seven semesters. Graduating seniors need a minimum of four credit courses in their eighth semester. If the student is using a college class or district approved night school as one of the credit classes, the student must:
    1. Bring a copy of their paid registration to be kept on file.
    2. Notify of pending ineligibility; the district progress reports satisfy this portion of state law for our district courses. These students must bring a progress report at 4-5 weeks in order to notify the student and parent of pending ineligibility.
    3. Provide grade report at nine weeks to check for eligibility.
    4. Submit final semester grade copy to be kept on file for grade check.

  6. DOMICILE REQUIREMENT - Except as otherwise stated in Article 15 A.I.A. bylaw, a student, whether an adult or not, is privileged with eligibility for interscholastic competition only at the school in the district in which his/her PARENTS are domiciled. Some exceptions:
    1. Ninth-Grade enrollment - A student enrolling in the ninth grade for the FIRST TIME may attend any member high school and be eligible for interscholastic activities. Upon any transfer by the student after the initial enrollment in the ninth grade, the Transfer rule applies.
    2. Legal guardian - a student for whom a LEGAL GUARDIAN has been appointed by a COURT of competent jurisdiction may be declared eligible by petitioning the A.I.A. ( Bylaw 15.5.3.1)** Students not living with their parent(s) may not be eligible and should be reviewed by the athletic director. Students with court appointed guardianship may need to be appealed to the A.I.A. prior to being eligible.
    3. Student did not paricipate in sports or a particular sport in the last 12 months - Review with athletic director for the use of A.I.A. form 530.

  7. NON-RESIDENT STUDENTS - Students not domiciled in your attendance zone should be reviewed by the athletic director if they:
    1. Are a ninth grade student enrolling for the first time in high school.
    2. Have been in attendance at your school for 12 months.
    3. Need a Form 530 to verify nonparticipation in sports or particular sport for 12 months.
    4. Have any other exceptions listed in the A.I.A. bylaws.

  8. RESIDENT STUDENTS - Students domiciled in your attendance zone will be eligible if they are ninth grade students registered for the first time in high school, have moved into your attendance zone, or have been in your school for 12 months. Any other resident students should be reviewed by the athletic director. An example would be a student that attended another high school and transfers back to your school while being domiciled in your attendance zone.
